The plan runs on a policy period, ending on the last day of the calendar quarter one year after the purchase date.
Plan applicable towards general dental services only. The plan applies up to the first $2500 of treatment in a plan year, thereafter, standard fees will be charged.
Renewals are available 30 days prior to expiration.
You have the right to cancel this plan at any time by notifying us in writing, however, the purchase of this plan is final and is non-refundable.
